For self pay clients*: I will provide you with an itemized receipt ("superbill"), reimbursement is not guarantied. Please contact your insurance company first to discuss your rights under the Affordable Care Act.

Anthem Google are typically fully reimbursed for lactation services, from my experience. Please check your benefits.

I am located in Cambrian neighborhood, San Jose and the areas of my service include the cities within up to 15 miles radius from my home, listed below
For any location that is outside of the 15 miles radius (but within the service area listed below) I will collect a $50 convenience fee that is not covered by insurance. .
