Insurance 

 

I am in-network with most of United Healthcare plans, but not all of them. I am out-of-network for UMR plans.  

  • I submit separate insurance claims for the lactating parent and baby (when present), as they are considered two separate patients. Both have to have UHC as their primary insurance at the day of service.
  • If your baby is not covered by UnitedHealthcare at the day of the service, an additional $125 fee will apply. A detailed receipt (superbill) will be provided.
  • If only the infant is covered by UnitedHealthcare and the parent is out-of-network, the parent will be billed the full consultation fee. Any reimbursement received for the baby will be refunded, less applicable processing fees.

 

For self pay clients**:

I will provide you with an itemized receipt ("superbill") under the lactating parent name using CPT code S9443, reimbursement is not guaranteed. Please contact your insurance company first to discuss your rights under the Affordable Care Act.

**Including but not limited to: Kaiser, Aetna, Meritain, Cigna, Anthem Blue Cross, Blue Shield Blue Cross, HealthNet, Blue Shield of California. ​

Aetna may reimburse my services at 100% as an out-of-network provider. Please check your benefits with your insurance company.

Meta employees covered under Meritain are typically reimbursed for out-of-network lactation services, from my experience. Please contact your health concierge for details about "Enhanced Care".

Google employees covered under Anthem plans are typically fully reimbursed for lactation services, from my experience.  Please check your benefits with the insurance company or company health care liaison. ​
